Welcome to the Chadwick School District’s Health and Wellness Resource Page. The purpose of this information is to promote the physical, emotional, and social well being of students through increased access to quality services, education, and information; however, it is not intended to take the place of seeking medical attention.

Medication is  given to students during school hours only when necessary to permit the student to attend school, and in compliance with school policy.  Medication is defined as to mean all drugs, including prescription and over-the-counter medication.  To request that we administer over-the-counter medication to your child you must complete our Health and Development Form.  If the mediation is a prescription, the parent or guardian must also complete and sign a Daily Administration Release Form for each medication to be given.  Prescription medication will need to be provided in the original container that is properly labeled.  Your pharmacist can and will provide a second bottle with the correct labeling needed for it to be kept at school all you need to do is ask.
